Record and Share Information
Inside the portal, patients can create a personal health record account, share demographic and clinical information, and send and receive secure messages – including the reminders, electronic copies of health information and clinical summaries required to achieve meaningful use.
SpringCharts Patient Portal is integrated to the SpringCharts portfolio of products using standard interoperability protocols, streamlining the flow of electronic information between your practice and your patients.
Functionality includes the ability for your patients to:
Track and graph their health measures including:
• Height, weight and BMI
• A1C
• Steps (pedometer)
• Blood pressure
• Medical contacts
• Calories
• Carbohydrates
• Blood glucose
• Triglycerides
• BUN/creatinine
• Cholesterol
Upload documents including:
• Legal documents including power of attorney and advance directives
• Medical records
• Financial records
• Personal documents
Print a summary of their personal health record.
Patients can:
• Send patient medical summary to their medical contacts
• Direct SpringCharts Patient Portal to send their personal health record to the other practice locations
• Preview their SpringCharts Patient Portal health record forms prior to delivery
• Store all sent health history forms in their SpringCharts Patient Portal account
• Provide your practice with a bar-coded “fax back” form so they can send health information to their SpringCharts Patient Portal inbox
